Please fill out the following intake forms and bring them with you to your initial visit. You may also arrive 15 minutes before your first appointment and we can provide you with the intake forms to complete.
We also ask that you to bring all of the paperwork, insurance information, imaging studies/reports and any particular clinical records from physicians that you have seen prior to your first visit. This clinical information will help us be as thorough as possible.

We are in contract with most major health insurance plans. Please understand that it is ultimately your responsibility as a patient to ensure that your carrier covers chiropractic services. We will gladly look up your benefits and provide you with an estimate, but be aware that your insurance company may deny payments and defer them to you as a patient. We will do our absolute best to give you the most up-to-date and accurate information possible, but please understand that healthcare plan benefits are different for everyone and constantly changing. If you have a question in regards to your specific health insurance plan, you may call Member Services on the back of your insurance card to confirm your chiropractic therapy benefits. What should I expect on my first visit?
One of our doctors will take a detailed history and then perform a thorough examination. A preliminary diagnosis will be rendered. The treatment plan will be recommended which is specific to you as an individual. Our goal is to resolve your condition in a timely fashion and avoid future injuries.
What should I wear for my visit?
It is recommended that you wear clothing that is best suited to allow for full range of motion with the ability to apply treatment to the skin.
How long are the visits?
New patient visits are typically one hour, this will include the history, examination and treatment. Follow up visits range from 30 minutes to one hour.
What is your cancellation policy?
We require at least a 12 hour notice for cancellations. Failure to give adequate notice will result in a late cancellation fee.
